Your registration is not yet complete! Before you can be qualified to participate in the Club Sports Program, you must submit the following forms* to your team President or officer in charge of collecting membership forms.
The officer responsible must turn in the membership forms for the team as a whole to the the Club Sports Desk.
The Desk is located in the second story of the University Union in room 203 near the stairs to the UU Plaza. If you need any clarification on our location please stop at the Information Desk for directions.
